What is a significant limitation of the laryngeal mask airway (LMA)?

The laryngeal mask airway (LMA) is designed primarily for use in unconscious patients or those with reduced consciousness, and while it has many advantages, a significant limitation is indeed related to high airway resistance potentially leading to leaks around the cuff.

When the intrinsic resistance in the airway is elevated or if the patient has anatomical variations, the LMA may not create an adequate seal. This can compromise ventilation and increase the risk of aspiration, as well as affect the effectiveness of positive pressure ventilation. Proper placement and cuff inflation are critical to ensure that a proper seal is achieved; however, in circumstances where airway resistance is high or the LMA is not well-fitted, leaks can occur, rendering the airway less effective.
